What this policy covers
This policy explains what happens to information you send us through this website, including the contact form.
It is not our Notice of Privacy Practices. That is a separate document describing how we handle protected health information as a healthcare provider, and we give it to you when you become a patient. If you would like a copy before then, just ask.
Information we collect
When you use the contact form, we collect what you type into it:
- your name
- your email address
- your phone number, if you choose to give one
- the best time to reach you, if you choose to say
- your message
We also briefly handle your device's IP address when you submit the form. It is used for two narrow purposes: to limit how many times the form can be submitted from one place in a short period, and to check with our anti-spam provider that the submission came from a person. We do not use it to track you across the site or store it alongside your message.
Simply reading this site requires nothing from you. We do not ask you to create an account, and there is no login.
Health information
Please do not send detailed medical history through the contact form. It is email, not a secure patient portal. To discuss your health, call us at (302) 555-1234, and we can talk it through properly.
We know people often mention their condition when asking whether we can help, and that is a reasonable thing to do. If you do, we treat what you tell us with the same confidentiality we give patient information: we use it only to answer you and decide whether we are the right fit for your care, and we do not share it outside the practice except as needed to provide that care or as the law requires.
How we use it
We use what you send solely to reply to you and to arrange care. We do not sell it, we do not rent or trade it, and we do not use it for advertising or add you to a marketing list.
How long we keep it
Your message arrives in our email inbox, and we keep it for as long as we need it to respond to you and to keep a record of the enquiry. You can ask us to delete it at any time — see below.
Service providers
A few companies process this information on our behalf, each limited to the job it performs:
- Cloudflare — hosts the website, and provides the Turnstile check that keeps automated scripts from flooding the form
- Resend — delivers the contact form message to us as email
- Google Workspace — provides the inbox the message lands in
Cookies and analytics
This site sets no advertising cookies, and we have no analytics or visitor-tracking software installed. Cloudflare Turnstile may set a token needed to confirm a form submission comes from a person rather than an automated script. If we ever add analytics, we will say so on this page first.
Your choices
You can ask us what information we hold about you, ask us to correct it, or ask us to delete it. Email info@firststatehomept.com and we will take care of it. If you are a patient, the rights described in our Notice of Privacy Practices also apply to your health records.
Changes to this policy
If this policy changes, we will update this page and the date at the top of it.
